Student Records Forms

Listed on the Student Records website under "For students in the Faculties of Arts, Science or B.A. & Sc."

  1. Graduation Term Change Request:
    • To be used ONLY when the Minerva Application for Graduation is closed.
  2. Late Course Change - Honours to Major:
    • To be used in order toÌýswitch from the Honours section of a course to the Major (regular) section.
  3. Major Concentration Change Form:
    • To be used only if you want to add Biology, German Studies, Hispanic Studies, or Religious Studies as first or second major concentration.
    • You must already have Biology, German Studies, Hispanic Studies, or Religious Studies as a first or second major concentration AND want to change/add/remove another major or minor concentration.
  4. Pre-Admission Transfer Credit Form:
    • To be used If you were granted a lump sum of transfer credits as "TRNS XXX" upon your admission to Â鶹AV Faculty of Arts
    • Submit this form to have these credits articulated (ie. to use them towards program requirements). Please consult the Arts OASIS University transfer page for instructions.
  5. Removal of Foundation Program Request:
    • To be used if you've earned 24 or more credits in your first term, and are seeking to commenceÌýU1 so as to select your major.
    • Grades for your first term of study at Â鶹AV must be available in order for a decision to be made.
  6. Request to Exceed Credits:
    • To request permission exceed the 17cr credit limit in a term.
  7. Request for Late Course Addition:
    • ​â¶Ä‹â¶Ä‹â¶Ä‹â¶Ä‹â¶Ä‹â¶Ä‹To request permission to add a course after the Add/Drop deadline.
  8. Request for Late Course or University Withdrawal:
    • To request permission to withdraw from a course after the Withdrawal without Refund deadline.
  9. Request for a Leave of Absence:
    • ​â¶Ä‹â¶Ä‹â¶Ä‹â¶Ä‹â¶Ä‹â¶Ä‹Your request must be made before the end of the withdrawal with refund deadline of the term in which the Leave is requested.

  1. Special Letter Request:
    • To be used to request a special letter regarding Â鶹AV’s accreditation information, semester dates, language of instruction, etc
  2. Post-grad work permit letter:
    • To be used to request a letter confirming graduation eligibility before your degree has been granted.
