The specimen shows fusion of two aortic valve leaflets, consistent with a congenital bicuspid valve.  (A median raphé is just evident (long arrow, (A)). Multiple small vegetations and foci of ulceration indicative of acute infectious endocarditis are present (short arrows).

No clinical information is available. The descriptive card suggests that the valve fibrosis is the result of recurrent episodes of endocarditis; however, trauma related to a congenitally abnormal valve is more likely the cause.
