How to get an appointment at The Neuro (Montreal Neurological Institute-Hospital)听
The Neuro is a referral only hospital, which means you will need to be referred by either your general practitioner (GP) or a specialist to get an appointment.听
All referrals must include the following information:听

Medicare or health insurance information听

Reason for consultation听

Phone number(s) to be reached听

Name and license number of your referring physician听鈥�
Appointment procedure if you saw a GP/family doctor:听
Please note as of 鈥疢arch 31st, 2024,鈥� only requests transmitted via the Dossier m茅dical 茅lectronique鈥�(DM脡) and SAFIR Web will be accepted at the CRDS.听
Your family doctor or general practitioner must send a consultation request to the CRDS (Centre de r茅partition des demandes de services) using the SAFIR online platform.听
Family doctors and general practitioners must use the MSSS forms for neurology or neurosurgery to refer a patient to the CRDS.听
The CRDS will then seek out an appointment on your behalf and then contact you with the appointment date.听
